Once your child has been selected for a class, please use the following list to prepare any information, documents, and visits that are required to complete admission into our program.

1.  PROVIDE COPIES OF THE FOLLOWING:

Birth Certificate for enrolled child

(Blue) Immunization Form for enrolled child

Proof of Residence in the State of Alabama (Driver’s License or State ID)

Social Security Card for enrolled child

2.  COMPLETE AND RETURN ENROLLMENT PACKET.

You may print the packet from this link or pick it up at the Foundations office.

3.  COMPLETE ONLINE ENROLLMENT.

A code will be provided by the office if you are selected for classroom enrollment. Devices will be available as needed.

4.  MAKE FIRST TUITION PAYMENT TO SECURE YOUR CHILD’S SPOT.

Families choose from two tuition packages and payment plans.

Plan A: $200 per year tuition paid $20 per month. First and last month tuition paid by June 1st is required to secure a spot.

Plan B: $100 per year tuition paid IN FULL by June 1st  is required to secure a spot at the reduced rate.

After-School Care: $160 per month tuition. One month tuition is required by June 1st to secure a spot (applied to last month of ASC tuition).

5.  SCHEDULE A REQUIRED VISIT

Contact Ms. Conaway at (205) 588-5454 to schedule a time to visit the school for us to meet you and your child in person. *IMPORTANT* This visit does not affect enrollment status for Foundations but is required to assist our teachers in preparing for your child’s arrival in the fall.
